Program Simple API - repost

Goal for project is to create a simple web page that uses UPS & Fedex Ground set of API to request transit time from a set default ZIP CODE to a manual entered destination ZIP CODE and return the Transit time. Goal for shipping clerk to have web page open and enter the Destination Zip code and hit submitt, thru the API it will return transit time for both UPS and Fedex Ground. No need for freight costs or anything else.. Needs to be programged in PHP. I have documention for UPS & Fedex API and have account with them to download them. I have attached an example design of what the pages needs to look like with a submit button.
