Project Description:
Hi, My name is Darren,
i need a form based php website built,
website will serve a community and requires individual log ins that are customer created
Design: simple light colors, elegant
function: forms to be accessed & filled in from a tablet device (ipad) & saved onto a database
to be accessed from a desktop "back in the office" with a print function.
i will provide scanned images of the forms to be recreated for the website to the winner
