Project Description:
We need one PHP file to be edited (pf-1.php)

Our business requires our customers to sign a contract. Currently we have them complete the PDF Located at http://www.easyfundraisingcards.com/EFC-Agreement-Form.pdf . However many people do not understand they can type into the PDF and then print it out to sign, because they do not fill it out online, they do not fill out all of the fields, or do so with Incorrect information, This causes us a substantial amount of follow-up time. Additionally, the file is very large causing issues for people with slower connections.

As a result we have set up a wordpress Cform that collects all their information(giving detailed explanations, requiring fields and offering help). When they submit the form, it places their information into a data base (for late use in house) and then outputs that data into the correct places on the contract for them to print out. As you can see from the example the data is being placed into that page already, however the programmer misunderstood what we were trying to accomplish and placed the data into the webpage using "input fields" like this <span class="input_field">_______________________<input type="text" id="full_name" name="full_name" value="{Your Name}" /></span>

(WE DO NOT WANT THAT)

We want to print the individual data fields into the page using <?php print $customer-name; ?>

The file is already calling the DB However we need to have the data separated out (using a loop I presume)
1. We need to have 40 fields of data pulled from a MySQL table/row
2. Assign all of the fields an individual value (like $customer-name)
3. Place those fields into the appropriate places within the document

It is important to note:
A. Bidder must understand Wordpress and how to use global $wpdb; (even though it is already in the file)
B. Bidder needs to already have file edited BEFORE BIDDING
C. Bidder will not be given access to Cpanel, Mysql or wordpress. (period) So if there is not enough information available for you to do the needed work

You will be given FTP Access to 1 Folder that contains the file (also attached), Images of the database so you can see all of the fields and also the structure (Attached), and a URL to Check your work http://www.easyfundraisingcards.com/fundraising-card-order-form?task=cform_print&pf=&sub_id=45 (before bidding make sure you will be able to do the work with ONLY these items)

This will take 10 to 30 minutes for a person knowledgeable in wordpress/cforms/PHP/Mysql

- If you need clarification on anything please ask before bidding
- if you ONLY KINDA KNOW how to write code DO NOT BID - this is not the job for you.
- We will not be offering a milestone until after the code it written
- We are aware that the code is not the most tightly written, we do have future projects available to tighten code, secure the data and build 'inhouse' administrative Mysql manipulation code.
