Project Description:
We need someone to create a new contact us page on our magento store. The page is to have the following on it:
1. We use Freshdesk as our helpdesk and need a ticket form to be placed on this page which uses the Freshdesk API to create the ticket.

2. We would like a simple drop down box which loads some text on the page. For example the drop down box would be called "My question relates to:" and have the values shipping, warranty, payment, general. When the value "Shipping" is selected some shipping related FAQ text would be displayed in a right column. This should just be a simple drop down box which is not required as part of the ticket submission.
