Project Description:
We need to create a 1 page PHP order form. Once all the information is filled in it is then sent via email.
The form has a few variables, but is very simple. Please see the attached mockup.jpg and the menu.pdf, essentially the form will be a method of ordering anything from the menu!
You will be required to input all of the items from the menu into the order form.
I would also ask that the form is kept simple so that it can be edited in the future as prices & items change.
