Please design a page in correspondence with the current web layout that you are currently working on.
Basically we need a request form produced. I’m assuming you would be doing this in PHP.
The color schemes are to be the same as the current layout. Each request is to be sent to a designated email address. Is there anyway of automating this so as the request come in they are directly inputted into an excel document based off the fields?
The page is to say at the top “Request a Free Gown Sample”
The request fields we will need the following fields.

** Country (If US Prompt for 50 states) (If other countries prompt a field to type in providence)
** State: (All 50 States)
* Zip Code:
* Telephone:

* Expected Number of Graduates:
** School Type: (High School / Middle School / Elementary School / Pre-School / Kindergarten)
** Gown Color: (Black, Maroon, Red, Gold, Emerald, Forest, Sky Blue, Royal, Navy, Purple, White)
Please set the following fields to be necessities so a request cannot go through without them; Name, School Name, Address1, City, County, State, Zip Code, Telephone, Expected number of graduates, School type, Gown Color.
The only field we won’t need as a necessity is the fax numbers.
