PHP Application Script

Hello, I require a php script using a mysql database backend that will do the following. Please be advised that payment is sent once the project is complete and has been tested on our server. -- There are 2 main areas, Staff and Admin. Staff will be assigned a username and can login (select there name from a drop down box and enter password) to submit customers information (application). We have a few fields that need to be filled in including name and email address. Once the application has been submitted the customer will recieve an email from us. Once a staff member has completed an application and submitted it they should be able to view a application number that can later be searched for in the admin area. The admin area will allow us to view all the applications and export applications to CVS format (for example, todays applications) - we need to be able to click a link and it will download a csv format. We also need selected users to be allowed to access the admin area, not everyone. In the admin area we also need to search for applications and view an application and type some notes about it (should a customer call). We should also be able to create new staff members and have a seperate page that we can view what staff are logged or not logged in and and how many applications they have made today and in the past. That is all we need. Budget is around $50-$60 and a quick turnaround is required please. Payment will be made once the project is complete on the 30th January. Thanks, Zack.
