This should be really simple for someone who is proficient in PHP
I need a simple file upload form to insert into a wordpress page template. I need the code for the form and a php file to upload to the server. Verifying that the form functions and any required troubleshooting should be included in the quote.
The form needs to:
1. Allow the user to enter the file path directly or browse to the file location
2. Require a password
3. Provide minimal security to keep anyone from uploading an executable file (or remote executing a file once uploaded)
4. Upload the file to a "customeruploads" directory in the website root.
