PHP Questionnaire Website

System for Questions and answer website. This system can be used as a Questionnaire or as a Testing website or maybe just as a way of getting feedback from website visitors. Also, please note this site will be used on a few different domains and as such needs to be fairly portable, I need to be able to upload the code to a new server - configure the mail server and domain settings in the admin page and everything should just work. First we need 2 Admin pages which allow the admin user to configure settings and create and add/edit/delete content for the home page and emails as follows : Need to allow admin user to set mail server settings for the domain this is to be used on. Our servers Require Mail authentication so we need open text fields for admin user to specify the senders email address and password and the domain name etc.. then ... Need to have text boxes for adding the standard text to the top of the emails that are sent out from this system. Need to be able to configure this text for each individual email address that is set. Then ADMIN Page for the web page content. 1. Home Page Introduction : [10 line text box] 2. HEADING : [One line text box] TEXT : [8 line text box] KEY STATISTICS : [5 line text box] QUESTION : [2 line text box] VISITOR RESPONSES: [One line text box allowing user to include answers like : yes,no or pass,fail or 1,2,3,4 these answers will be displayed on the home page as a dropdown menu for the visitor to select their answer] CORRECT ANSWER : [One line text box] TEXT TO EMAIL : [5 line text box] EMAIL ONLY WHEN : [Every time / correct answer / other answer {text field}] EMAIL TO : [Tick Box - Person Completing Form], [Tick Box - Text Box for {email address}] , [Tick Box - Text Box for {email address}] (This point 2. needs to repeat as many times as the Admin user requires. - eg/ they may have 5 questions or they may have 15 questions) The "front end" home page of this website needs to display all of this content and then a form at the bottom of the page for the visitor to complete with their First Name, Surname, Postcode and Email Address. This info needs to be added to an MS Access database together with their responses to the questions and date, time and ip address of the vistor who completed the form. After submitting the form the visitor then should see new page : split the page into black text on pink column on left (LEFT HEADING TEXT) and black text on lightest blue (RIGHT HEADING TEXT). Display all questions on one long page pls (question on left and answer on the right) These headings need to be configured from the Admin page also please. FOR ADMIN We then need a web page to display the content of the database in summary list form (showing Date, Name, Email, and a link from each record in this page "FULL DETAILS") this link will allow the admin user to see the reponses from each individual in full page. For the list summary view, please break the page into groups of 40 records per page and NEXT or PREVIOUS etc.. allowing Admin user ability to navigate through the reponses. The responses need to then be emailled to the entities that are set in the last part of point 2, and ONLY sent in accordance with the rules set by the admin user. (So for example send this email to the person who completed the form and maybe a second email address that they enter manually (like the admin user's own email address) and the info from [EMAIL TO TEXT] for each question together with the visitors answer should be included in the email. Finally we need a "front end" results page for website visitors - which shows a histogram of how many people scored from 1 to 10 - (*10 should be displayed if the number of questions is 10 :) ) This histogram (or graph) also needs to display small next to the header image at top of the page on the home page. That's basically it! SERIOUS DEADLINE - This needs to be completed by Sunday 22nd June Midnight my time (GMT+10). IF YOU DO BID YOU ARE AGREEING TO THIS POINT - Your failure to deliver this ready, tested and bug-free by that time will really let me and my client down and you shall be entitled to receive only half of the agreed bid amount.
