Project Description:
Need a simple form created in php. When submited entries are stored in MySQL database.
I will provide server environment, and phpmyadmin admin page
1)Pull down to select which person is filling the form. This pull down should query the database.
2)14 yes/no questions.
3)A time stamp for when submitted.
4)A report page to display all submission organized by name and date.
I will make a mock html page and list the entire yes no questions for you to follow.
