Zen Cart Database problems

I have been having multiple issues with my website, which is built on ZenCart. I need someone who has experience with ZenCart to repair some issues. I make custom pet tags (and other personalized items). I have text boxes set up for the customers to type in their personalization details and instructions before placing the item in their cart. I also use radio buttons and drop down boxes to help the customer select the personalization details. As far as I can tell, all of the drop down boxes and radio buttons are working. However, when the customer types their details into a text box, their information isn't coming through the last step of the checkout process. If they've typed in their pet's name & owner information into the text boxes, the details only display as TEXT on the last step of the shopping cart and in their order confirmation email. I am also noticing that's happening in the text box for additional instructions. I believe this is happening on ALL products, not just the pet tags. My site crashed about 2 months ago. I was finally able to get most of it restored, but for about a month now I've been having this problem and can't seem to get it resolved.
