Project Description:
We have a registration form that is paper right now. It is six or seven pages long and consists of checkboxes and text fields. We need to build a php web page that will allow people to fill the form out online. We want to be able to print each record seperately so it looks just like the form does - essentially duplicating the paper form. I want all the data stored in a database so I can search on just about any field.
This database should be searchable; we also need a few simple reports on this data. Maybe four reports that lists people by city, by county, etc.
