simple database- and php-work

I need a php-coder who can create this. I have a file with email-address and name. I need a MySQL table with this information. You have to create a example how I can add information into a database. Then I need a php-script for removing people from that database. When a customer clicks on a url (with inside their email-address) then they will be removed from that database and send a small standard-confirmation. A simple job for someone who knows what to do. I haven't to much time; in a couple of hours it should be ready.
