Form To Email on PHP4 Server

Project Description:

I need a form to email php script made with simple inputs such as Name, Phone, Email, Comments that will then send an email to 2 email addresses upon hitting submit.

I will provide an ftp login for you to work in and test, with an initial skeleton form template in the ftp directory. there will also be a copy of phpmailer in the directory should you choose to use it.

Upon hitting submit i would like to automatically direct the visitor to a "thank you" page.

The form should also check for a valid email address and will not let you submit without name and at least email address.

i would like the email to be from the input email address and be able to change the subject line. For now lets just have the subject line as "Form to Email Test"

The shared server has php version 4, I don’t know what php libraries they've enabled, and if you have to send the email via stmp email, it will have to send encrypted, password protected to/from gmail.

This should be a rather simple project and ill likely select the project person before the 1 day time is up. thanks!
