Shopping cart help Need a simple online orders page

I am looking for a simple shopping cart order form page for one of my services that I supply online. We sell realestate video tours and need an orders page to allow our client the ability to order our tours from our website. I currantly have a merchant with the ablitily to accept credit card payments online but i can only accept pre determined amounts. I need the abilty to have different options to add to each order for instance;

The base price of our tours are $189 per tour upto 2500 sq

then there are extra option:

$15 per 500sq over 2500
$60 voice over
$25 mls Pictures
$15 to upload to youtube

etc.

So I would like the client the ability to choose and customise ther order and then create a total of all there extras services ordered then proceed to payment checkout where the payment will be made online using my merchant.

Also I would like for there to be a email notification whenever a tour is ordered. Also I would like for there to be a promo code section for disounts where there will be special promocode to enter for the discount.
