i have a website and i need some one who can write me a page with php script that allow internet users to upload their own single or multiple photo to my email with their personal informations such as their name and email address.when they on the summit button, it leads to a page where they can continue to pay me with paypal.
nelow is an example website of others which i am trying to do.

i think this is pretty easy for people who know php well.
