MS SQL Database extract of data to CSV format

Project Description:
We have a MS SQL Database that is 30GB in size. We need to extract the following data from the database for the period between July 2006 and June 2009, broken down by quarter, in CSV format. We need for each order during that period:- Customer name, keycode, shipping and billing addresses, order number, total price of order, taxes, shipping, items on the order
We need this extraction of data as soon as possible. The MS SQL .BAK file is stored on a FTP server for download.
