Need a Microsoft Word / Openoffice / Libreoffice Macro Guru / Macro Guru

I have many large sized HTML files with simple HTML tags. Microsoft Word 2010 can read the HTML files and keep the HTML formatting beautifully. I can also save the HTML files into docx files without any problem. Now, I need to build a Table of Contents ('ToC') and multiple Indices. The ToC should start from the 2nd page of the document. The indices should be at the back of the document. The indices should be built by using "Mark Entry".

Attached are 3 files, a HTML file, a docx file of one of the HTML file and a docx file with the ToC and indices. The ToC and Indices are created by myself manually. I need a macro to create the ToC and the indices automatically. Further, you need to provide me a script (VB, PHP, PERL, etc), such that I can convert the HTML file into a docx with the ToC and indices by using command prompt. Therefore I can simply type at the prompt "convert input.html output.docx" and have everything done without Microsoft Word being opened. I do not mind if Microsoft Word, Openoffice or Libreoffice is being used.

In the docx file, the categories that need to have the indices are: 1. ToC, 2. IPC, 3. UPC and 4. Elements in the diagram (but exclude the numbers). The 4th category, Elements in the diagram, has not been created in the docx file.
