CSS Markup and Design for Dynamic Form

Convert form table design to CSS 2. Eye for graphic design required. I have ten form templates, many sharing similar elements. This job has two parts. 1. Markup: Finish converting existing tables to fieldset/css markup. See sampleTableToCSSMarkup.htm. 2. Design CSS rules for layout. This is a little tricky: worksheet will be dynamically generated from different combinations of elements. Fieldset elements need to flow well and scan well, but frequently used elements should be consistently placed for usability. I need someone who can handle the technical elements with minimal supervision but also take initiative with design considerations. Code needs to be as simple as possible and WELL DOCUMENTED AND NEAT so I understand and modify tags. Css design and code samples required with your bid. Project must conform to broad range of browsers – no leading edge fancy tricks.
