I am looking to have an awesome online registration form designed & coded.

The information added here is automatically added to an online database using the "post method" / "action" form details contained in formjavascriptcode.txt
This current version of the form unfortunately cannot be edited or accessed by myself - hence the need to have a new form created which is much better looking but VERY IMPORTANTLY uses the same javascript validation requirements & "post method" / "action" as contained above - so that all information continues to be added to the backend database.
You can see a "theme" mockup at mockup.jpg and further design "feel" on form1.jpg / form2.jpg to give an idea as to the look / feel of the form I am going for.
1 - The form needs to usethe same javascript requirements & "post method" / "action" as contained in the original form so that it correctly posts to the database
2 - The existing data input validation requirements need to be included so that the appropriate validation error messages still show ( e.g. First & Last Name are required fields etc ) see
3 - Required form fields can be seen in formfields.docx
4 - Once submitted - the form currently redirects to "http://www.collegeofmagic.com/thankyou.html" A separate submission confirmation page must thus be created matching the "look and feel" as the main form page with the message "Thank you - your registration form has been received."
