I need a one page website that has a similar structure to the one that is current up: www.heidimccaulley.com. Her picture needs to be dominant, she is selling Home, Life, Auto insurance and Annuities. In the very center, there needs to be a form asking for Name, Address, Zip Code, and date of birth. The form should be functional and attractive. Take a look at the website and you will see the concept. I just need it to look better, cleaner, and load faster.
