Landing Page / Squeeze Page Templates Needed

I'm looking to create several landing page templates in different styles in HTML with graphics. 3 Landing queeze Page with opt-ins 3 Landing Affiliate Pages with affiliate buttons Please see attached document for the layout and example: There are really 3 sections to each template: Header - top area where you can write a headline and put a picture or graphics Content Area - where you can embed video or write text Call to action area - where someone would opt in with an email or click a button for an affiliate link. I need this done right away. Again, please refer to the simple layouts I've shown you... I need a good price and fast. If you already have these pages built, then let me know. Templates and images mush be royalty free, and YOU MUST SHOW EXAMPLES OF YOUR WORK AND EXPEREINCE CREATING SQUEEZE PAGES AND LANDING PAGES IN THE "PM' (PRIVATE MESSAGE AREA" Again...
