I have made a web form using coffeecup. here is the link: [url removed, login to view] I would like to have this as my own css style sheet that i can use with dreamweaver CS6 that functions in the same way. I receive a very clear email when this is filled out. I will send to you a copy of the email I receive so you can see how the information arrives to me. In the CSS form I would like to be able to edit the background colour and add a main image at the top as in the form i have made. There are check boxes that I would like to add or remove, but other field would be standard. Thanks
