Project Description:
I need a simple PSD converted into 3 VERY simple responsive templates. Attached are images of how i would like it to look on a browser and a mobile device this is an image of the main page I need the two choices will be links to two pages one will have a form with Name, Phone, eMail, Date of Experience, a text box for 'tell us what happened' and submit button that will go to just a page that says thanks for your review. i will need that form functional. and the other page will just be some text then a list of links where people can review. i need to beable to add as many links as i want with a description below each.
