Project Description:
We are a small company, less than two years old. I use supremecenter54 as a host for my site. It can handle PHP, and Mysql.
We already have a website, but what we would like to have is a customer/new customer login, where they could request things like RMA's, check the status of their equipment already submitted, view reports, etc.
If the customer is new and requests service for a particular instrument, he/she fills out the following to create a logon name and password.
Company name
Company website
logon name
email address
Model number
serial number
options
Once I receive this information and they decide to go ahead with sending the instrument in, they would then fill out the rest below to receive a RMA (return material authorization)

There might be more (but not alot more), but I am unsure at the moment all that I will need.
There might be more work on sharpening up company logo and website.
