I have a WordPress website (with a purchased theme) that uses the Newsletter plugin (http://wordpress.org/extend/plugins/newsletter/). I need the plugin modified to:
1. Show two fields dynamically ONLY if the visitor answers no to a particular question.
2. small CSS modifications to change spacing (see attachment).
3. The form results are added to a database. I need you to ALSO have the data emailed to a particular address.
