Project Description:

I need a survey based web site with a good content manger to;
1. View all questionnaire results
2. Manage users
3. Send email (multiple or single recipients) invites to users to participate in survey
4. Add more questions as needed or update existing
5. Admin must have ability to assign answer format to each questionnaire; Consider answer formats below;
* Multi-Point Rating (e.g. Strongly Agree to Strongly Disagree)
o Option to Edit/Specify rating labels
o Include/Exclude "Unable to Rate" option
o Low to high or high to low rating label direction
* Semantic Differential
o 5 rating points to 10 rating points
o Include/Exclude "Unable to Rate"
* 5-point rating with comment
* Multiple choice
o Select one or select multiple
o Limit number of selections ("Select up to X choices")
* Drop-down selection list
* Ranked choices
* Short answer text box
* Single-line text response
* Numeric values (%,$,etc.)
6. Ability to export raw data to MS Access DB while maintaining data integrity
7. Web Site and DB must be secured ******
8. Other criteria may be added as your expect input is highly appreciated and valued.

May use PHP, MySQL.


I will consider a completed survey web site project only if minor modification is needed to meet my requirements.
