We are in need of someone to create a small event registration portal, we currently do event registration using two pdf forms, one for the lodging / rooms and one for the tickets for the event. Once these forms are filled out they are faxed or mailed to the office, we would like this done all online.

The vision, which we are open to suggestions is creating a web portal (we provide access to cpanel hosting w/ mySQL)which will need to have a front end and a back end. The front end is a simple web form that allows users to put in their registration information, this form will need to calculate a total based on how many tickets / price levels requested. This form will ask how the client will be paying but will not take any CC info! Once user clicks submit this information needs to be put into matching fields in a mySQL database, an e-mail needs to be sent at an address specified, and finally a confirmation e-mail sent to the user saying thank you.

The back end needs to allow us to set price levels for tickets and manage the form, in addition it needs to have the ability to show / print clients registered for the event (mySQL query).

I have recent registration PDFs that will be provided to be used as a guide.

If you have a similar system built that can be adapted we are open to discussing it.

We are on a deadline and appreciate fast but quality work.
