PHP Upload Form, upload Mp3, JPG, post infromation to mysql

Project Description:
We need a php script that will allow a user to upload all there contact information which will be written to a mysql database a account will be opened for that user. The user would then be prompted to create a new album on our site. The user would then be prompted to upload song names and mp3 files for that album. The mp3 files would be uploaded to two places. The first location would be a on our download server (which is out side www). The second location the mp3 file would be automatically turned into a 30 second clip starting from the begining of the mp3 file and stored on our server (www/samples). The album name and song names would be stored in our database. The user would then be prompted to upload a jpg image file (cdcover) which would be stored on our server. After successfully compleating the account creation and upload the php script would email the user of the location of there album on our site.Please email me with any questions.
