We have two forms for uploading PDF files, and Video Files.They also update a database with the file information.
Here is what we want to accomplish.
The forms work but...
We want to secure the forms, so they only allow specific file types. In two stages - both with JavaScript that pops up a warning if the file type is wrong, and by PHP to deny the wrong file types from getting into the system.
We also want to add to the database table a timestamp to indicate when the file was uploaded, and an IP address field to indicate where the file was loaded from.
All form entries must be protected from possible SQL Injection attempts as well.

I can limit uploaded files by extension. Will validate via Javascript and PHP (client side and server side). Also I will validate content too. So, no way to rename "test.exe" to "test.pdf" and upload it. Also I will create MySQL database with 1 table. And record all uploads into this table. File name + IP + timestamp.

Will verify the file extension, size and MIME type via
PHP and make the alerts through Javascript. PHP will
be used to avoid MySQL injection as well.
