Database building

I am looking for someone who can generate a database from an online SQL query system.

The site works like this:
From a droplist menu you choose a state, then, from another droplist menu you choose a county, then, from another droplist menu you choose a town and then the system shows about 30 fields of data for that town. About 15 data fields are fixed field, while the rest of the data come out as a table in html form.

I need to put the fields in a database form. That is, for everyone of these “queries” I need each of the data fields (including the ones in the html table) to be taken to different columns of a database, so that each town query becomes a row of information in the database.

The end result can be a database in excel, access, csv, txt, dta, or any form that I can later take to a statistical program like STATA. I attach an example of a query and the end result I need for that query.
There are 3,000 queries to be done, I will send you a list with the names I want you to look up. I need this done in at most 2 weeks.

All deliverables will be considered "work made for hire" under U.S. Copyright law. The Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).

Under no circumstances will the winner of the contract be allowed to remove any data from the server to another server, or copy data for any purpose without authorization of the buyer.
