I have written 350 page book on Health topic.
I would like to covert it into an e book format for the kindle and iPad.
I have used several compliers, but they don't retain the format for text and tables including headers and footers.
I would like someone who has the expertise in covering the PDF book into an e book that can be uploaded to Amazon kindle store and iPad version.

I have included a sample chapter to see if you can convert to kindle version maintaining the formatting.
