Create a Joomla Component that auto creates a pdf from form

I need a coder to create a joomla 1.5 and 1.6 compatible component that will allow the super administrator to create a form on mainly yes or no questions/and/or/ to select options from a list. The administrator will then state the answers. Let me explain. for question 1, if the registered user ticks "yes" then certain pre-written text will be auto generated into a pdf and emailed back to the administrator. If the registered user ticks "no" then a certain different pre-written text will be auto generated into a pdf and emailed back to the administrator the component must also grab the registered users full name and place it into the pdf. It will be a type of assessment questionnaire, that, according to the pre-written answers relating to each question, generates a pdf with the assessment in it. another example. the registered users full name is John Black. Question one reads, "do you like to read" with the options of yes or no. the user ticks "yes". The pdf that then gets emailed through to the administrator will read:

Dear John Black
(prewritten text that doesn’t relate to any questions= Thank you for taking our assessment at Business Name. Please find you assessment below)
(there will be a paragraph break between each question)

Seeing as you enjoy reading, .... bla bla bla

(next answer to question here)

If he had ticked no, then the pdf would look exactly the same exept that "
Seeing as you enjoy reading, .... bla bla bla" would now read "seeing as you dont enjoy reading,... bla bla bla"

The component must allow for an unlimited amount of questions, and allow for different assessments (so the questions must fall under the assessments).
