The code below does what you want:

$resFromName = "CompanyName"
$resFromEmail = "email_at_address.com"
mail($fromEmail, $resSubject, $res,
"From: ".$resFromName." <".$resFromEmail.">")
You can edit the text message in the text.txt file and the rest is done by the script ( I have omitted the semicolons since they are not allowed in this text box.
