An app that can be used by a field foreman to enter in basic customer and site information to produce a work ticket than can be reviewed and signed digitally in the field and emailed to a variety of personnel.
The app should have a resemblance to the included Excel file, in terms of appearance and functionality.
The intention of the app is to be able to have an administrator vary the equipment list, worker list, and hourly price, so that field foremen at different job sites can use a mobile device to complete a form, have the customer review the pricing, sign for acceptance, and email off to various people.
Work Ticket #: A user-entered number for the work ticket.

Type: drop down list (that should be able to me modified by an administrator) to initially include Construction, Production, Drilling, Completion, and Other (that can be entered in manually)
Employee Name: should be able to be entered in. Also, would be beneficial to have an entry form to keep employee names, so a drop-down list of common employees can be used.
Position: list of positions included in second data tab on spreadsheet. Should be able to be modified by admin, and when selected from a pull-down list, automatically populates a corresponding hourly cost (which should be able to be modified by admin)
Start and Finish times: entered by user, automatically calculates time rounded up to next 15 minute interval.
Employee initials: signed with stylus, finger, or entered as text
Price: hourly price for type of worker
Amount: total amount based on price and hours calculation
Equipment #: will be entered in by user
Equipment type: dropdown list of equipment that can be modified by admin
Make Model: entered by user
Total hours/quantity: entered by user
Price: hourly price for equipment type - automatically populated when equipment type is selected, and can be changed by admin for occasional price updates
Total: hours * hourly price
Trucking Total, Material Total, Rental Total, and Fuel Total may be user entered.
Crew total should automatically calculate.
Equipment total should automatically calculate.
Subtotal should automatically calculate.
"Tee Bench Fee", should be a box selecting YES or NO. If YES, then 10% of the subtotal will be added to the total.
ND Sales tax: user entered if applicable.
TOTAL should add together all fields.
Shades fields for Well Name, Charge to AFE No., etc.: these will ideally be able to be hand-filled out using a style or finger by the customer in the field. As a backup, they can be filled out as typed text. This field may or may not be filled out, but should be readily available.
Signatures: need to be able to be easily signed and dated by field personnel.

The form should be able to be saved completed as a PDF, and be able to be emailed out from the iPad when complete. Additionally, ideally an office administrator should be able to modify any mistakes if necessary, to the originally filled out form.
This form will only be used internally by company staff. It will be utilized by field construction personnel. It should be able to have certain prices and positions modified occasionally as necessary by a trained admin user. It should hopefully be able to be used by multiple devices on different job sites.
