I need you to provide the creation of a Survey iPad App for use at a single Advisory Board meeting that will take place on January 25, 2013 in San Francisco, CA.
the development of one survey app that will provide the meeting attendees with an opportunity to give responses to multiple-choice questions. This survey will be created as an iPad application only, and will not function on other devices. The app will include approximately 10-12 slides of case study content provided by me in PowerPoint format. Attendees would answer approximately 4-5 multiple-choice questions (interspersed among the content slides), and also have the ability to fill out a form with personal information such as name and email address. The answers and information will be stored in the iPad app and transmitted to an online database via an Internet connection. We will supply the 4-5 trivia questions, and multiple-choice answers, to be used for the survey. We also will provide the required fields for the users to enter their profile information, if this feature is required.
