Project Description:
i need a service report with a scroll down list with 2 check boxes next to each line of writing. an ok check box with a tick and a not ok check box with a cross.
when the not ok is selected i would like a window to appear that can be written in (the fault). will also need a window where the customers details can be added and also saved for future work. each new report will need to generate a new report number. once all the boxes have been checked and fields filled in i would like it to be able to be printed or emailed in an A4 paper format. please look at pictures to see what is being used now in paper format.
