Giveaway Entry Form that limits 1 per user

Project Description:

I am looking to do giveaways on my site that is using vbulletin 4. I would like the form to only allow 1 entry per user, per giveaway.

I would like to capture the following in the form.
- first name
- last name
- shipping adress
- shipping zip code
- shipping state

I would like this form to be a plugin on the back end of vbulletin so that I can configure the options of the giveaway as follows.
- giveaway title
- giveaway discription
- value of product
- color options of the product
- amount of times the form may be submitted (limiting a certain amount of items to be given away)

In the backend I would also like to have a report showing the information gathered in the form and the ability to export it in a manor to be shipped using usps webtools.
