Magento 1.3.2.4 Losing information - Passwords - CC info

Magento 1.3.2.4 SilverThemes Tube Theme, OneStep Checkout.

Last week the site lost the backend passwords. Forced new default password and logged into the backend.
The backend user passwords were there, but I was unable to login using any of them. Had to delete all user passwords and re-enter them.

Now, when a customer tries to reset their password, the site returns an email with no name and no new password. The email is empty except for the template. Also, sometimes the checkout will not accept the zip code, returns error.

Also the site is modified to show the credit card number on the order conf. Now the CC number does not show at all, blank.

The problem is starting to grow, suspect the database is corrupt.

1. Find and fix why name and PW reset is not working.
2. Find and Fix why CC number is not showing.
3. Add CC verification number to order conf.

MUST HAVE PROVEN EXPERIENCE !!! DO NOT SEND RESUME !!

NEED PROBLEM SOLVED NOW!

This should be an easy and fast fix.
