Fill in a Spreadsheet with Data

We have around 150 pages of fuel consumption transactions that need to be entered into an Excel spreadsheet from a PDF. There are about 25 transactions per page, so total transactions would be in the order of 3,500-4,000.
There are four (4) columns that need to be filled into the spreadsheet for each transaction:
1. Date (dd/mm/yyyy)
2. Mixed alphanumeric (6 characters) e.g. ABC123
3. Floating point (typically 5 digits with decimal)
4. Currency (typically four digits with decimal)
About 95% of the source PDFs have been converted from another program (e.g. Excel, Word). The data on these pages can be copied and pasted directly into Excel; however the data do not resolve into columns. Instead, columns are transposed into rows and the resulting data cannot be easily formatted into the original configuration.
The remaining 5% of PDFs have been scanned from paper and copy/paste does not work.
Options to convert the data from PDF to Excel include, but are not limited to:
1. Direct entry into Excel
2. Copy/paste and reformat
3. OCR
We are seeking quotes from capable parties to convert all information from PDF to Excel in the most expedient fashion.
