Project Description:

We have three sites all using the same mod_ampcontact module but we need it modified OR a new simple contact form module created which will allow us to collect the necessary information.

You will be expected to modifiy the mod_ampcontact module on each of the three sites OR create a suitable quick contact form which will collect name, telephone, email, message (which it does already) and additionally three checkboxe options for best time to contact "9am - 12noon, 12noon - 5pm, 5pm - 9pm". Form will then be emailed to all administrators automatically.

All three sites have Breezing Forms component installed so this could be used.

We are busy with other jobs so cannot do this simple task, $30 maximum spend on this project.
