Web Form w/ js validation + Ajax upload script + PHP parsing

hi, i have a small form that needs to be filled out,verified, and an html order page to be generated at the end of the script with all the users form information, and emailed to the client so he can check out the html order form.

media is required to be uploaded to server in gif/jpg form with 2mb limit, through ajax upload script, and the url of where that image was uploaded to is passed as a PHP variable which is echoed in the HTML order form.

the fields are set up, they each already have a name, and a $_GET[''] as their value in case a user wants to go back. a javascript verification script needs to be in place to make sure certain fields are not NULL & image upload = successful. then once again as i already stated, the final information after user presses submit should generate html order page that is emailed to client.

the form is set up right now as a 2 step process.
1) personal information is filled out & 1 picture then submits...->
2)then user uploads more pictures depending on either 1,2,or 4 pictures he chooses and enters data for those particular pictures. --> submits -->reviews and submits order--> html page generated --> emailed url to client.

this is all really simple, i have experience with all this, and i will be able to guide you through this, just need someone to do the coding because i have no time to do it myself!

i can provide the ajax upload script myself you just need to make it work for my site!
