Contest Brief

I have a word document that I will upload.

1. Convert the Word Document to Html form and use css to make form look exactly the same as the word document.
2. Provide 2 versions of the new html document, 1 with input fields where questions are asked. The other page will show database entries from the database.
3. Allow the page to be downloaded as a PDF.

** Extra points will be given if you use gravity forms pluggin.

First one to get it done with accuracy wins.

Also, additional work will be offered to the winner.

  completed html should look exactly like the completed form as if they filled out the word document and saved to pdf. The final html of completed application and pdf download of exact copy is the most important piece of this project.

The output needs to look exactly like the completed application that was uploaded. As if someone actually filled out the word document.

Please, clarify something to me. Do you want the output with some hardcoded random values, or do you want to output the same entries from the input version?

  It is not necessary. We will put in code like, "Hello, [firstname], how are you doing? I see that you are [age] old."
  This contest is only for the design. And to download the final with sample values in pdf as well as html.

1) On the input version, please confirm that you want all the text and the form on the same page?
2) On the output (database display) version, do you want the explanatory text as well?
3) On the output version, do you want the information in entry fields so it could be amended and re-saved?

  2. the only difference to input version is to have the answers in place of the inputs.

Hello, i have one question. Do you want database in this project? You need a page that shows the information that is introduced?
We sure can do this job, but with BD included i think that this isn't the best payment.
