I need a simple logo for Summerville Mediation Center

I would like to see "Summerville" in a graceful, elegant script.

I would like to see "Mediation Center" in small caps (Bodoni font) beneath Summerville.

I would like a thin solid line immediately above and below "Mediation Center."

I need it all one color, black. I would like vector files, jpeg, ai, pdf
