Contest Brief

We need an Excel Macro made: Goal: Automate a process to take information from a source file and output it in the format provided in the outsource file.
Included: Four .xlxs files, as follows:
TRIADTESTSOURCE.xlxs
TRIADTESTOUTPUT.xlxs
TRIADTESTDATAMAP.xlxs
TRIADTESTPRICELIST.xlxs
Take the attached TRIADTESTSOURCE.xlxs) and manipulate it into the TRIADTESTOUTPUT.xlxs file using the TRIADTESTDATAMAP.xlxs and the TRIADTESTPRICELIST.xlxs.
The Data Map is your key to where the source information belongs in the output file.
The Price List is your key to getting the charges and the descriptions relevant to each medical CPT code in the source file.
The CPT Codes are 5 digit codes. There may be more than one CPT code on any given row. Each CPT code needs its own row so the corresponding charge/description will be on its own line. Additionally, the credit will need to be on its own line as well. So, at minimum, each row in the source file will have two rows in the output file (charge, and credit).
We receive source files like the one in this project that need to be manipulated this way every time. The amount of information in the file varies. So, this process needs to be able to work no matter how many rows of data are in the source file.
The winner of the contest will the the contestant that provides the stipulated output, exactly as requested, the quickest.
The winner of the contest will receive $300 with potential for steady contract work on future projects.

    The project was a contest, also a search for contractors for this type of work in the future. We were looking for speed & accuracy with minimal questions required for completion. It has proven to be successful since we have essentially received 2 winners of this contest.

    For Column AT in the output file, if the CPT code is given twice in the PRICELIST File then which description is to be considered. For example check CPT code 11440.

    What will be the format of Column C when it contains the middle name as well. Please provide an example. Secondly, In column D can you please explain how D4 will get populated in the output file?
