Contest Brief

I represent a brazilian Medical Instituition that works with general ultrasonography, mammography and bone densitometry (DEXA, DXA). I am looking for a FIDELITY or LOYALTY CARD with a clean, clear and very professional design.
The designer can produce the card in english first, afterthat we will translate the information to portuguese and ask the choose one to remade this in portuguese. So, I am demanding this art in portuguese, not in english.
This card must contain the address of the institution, phone numbers and the sentence: "CARTÃO FIDELIDADE" or "CLIENTE PREFERENCIAL" or both.
I would like two options: one-sided and two-sided card.
I attached to this description the logo of our business, one of our papers design which we use to delivery report to patients, and some examples of cards of another business that can inspire the designer.
