I want a sales page to collect leads like this: http://www.singlegrain.com/services/ for my WordPress website (http://bit.ly/1bm1Pwd)

I like the second background under the menu and the "Free Phone Consultation" right down to the bit where it says "Case Studies Available on request"
I want this to be created into a WordPress PAGE TYPE that I can then later create multiple ones off.

Please create a design that I can get coded into my WordPress site, that is in keeping with the existing site/theme colours and fonts.
