This has been customized with a banner or header you can see there. However, I am still losing 90% of people who click on the order form but don't purchase. I would like to test some new headers, keeping the rest of the design the same.

Header Image: Must be in .gif, .jpg or .png file format. Width must be 760 pixels. Height may be anywhere between 40 pixels and 170 pixels. Image must be under 500kb in size.

Please read the main sales letter carefully (http://alcoholfreesociallife.com/maincourse.htm) and come up with a header to replace or test against the one I currently have on the Order Form, to hopefully increase the conversion rate. Remember, your work needs to convince someone to keep going with their order. Feel free to use text, images, graphics etc. It just has to be as per the specifications above.

Please also try using the image of the book cover. Most people will not have seen the Alcohol Free Social Life logo before but they will have seen the book cover.
