Contest Brief
Here's a simple description of what is needed, I can give a more detailed approach when the bidder is confirmed.
Language : PHP + Codeigniter
Frontend : JQuery (any version)
Database : MySQL
Dynamically create forms relative to the selections a user creates. Then the user can view this dynamic form & input values into it. After this, they will be able to go to a final view page and view all the values from what was submitted from this unique form.
Simple Description.

If you see the attached file you can see the details regarding this. In short.
The First page has some "collections" of elements where a user can click on them and select which ones they want. In example. Collection 1 & 3.
It then looks in the database for collection 1 & 3 as to what the form input types are, names of the inputs, and required specifications such as minimum length and maximum length of the inputs. It takes these details and associates them with the unique "id" we are creating at the moment. This is put into a seperate table according to the "id"
Now when we go to let's say /form.php?id=10 it will pull up the specific inputs for this form. If it was /form.php?id=11 it would be different as to what the user selected for those.
On this page you can take all of these input fields and fill them out and submit them to another table called 'results'.
Now our final page is the view of everything. It loads all of the input fields relative to the id of the collection and the id of the user who submitted their information so let's say /view?id=10&userid=35
