Attached is our source code.

Tasks:
1. Require Telephone Number. If user leaves it blank, say "Please enter a telephone number."
2. If Question1=No, un-gray Question2. If Question2=Yes, un-gray Question3.
3. Make Question1 Required. If user leaves it blank, say "Form incomplete. Please go back."
4. If Question2 or Question3 become un-gray, make them required. If the user leaves them blank, say "Form incomplete. Please go back."

Deadline for this project is Tuesday 9am PST (west coast USA).
Please be available on skype Tuesday at 9:30am PST so we can test it on our live server. We will only pick a winner if they are available on skype at that time, so please watch for my skype request.
